Title: Hospital Liaison
Location: Southborough, MA

Schedule: Tuesday Saturday 9AM-5:30PM with flexibility

Responsibilities:
- Collaborate with hospital case management and continuing care teams to plan and coordinate discharge for home-based services
- Educate patients and caregivers on proper use, safety, and maintenance of respiratory and medical equipment
- Facilitate the delivery and setup of equipment at the patient s point of discharge
- Manage all required documentation, including intake forms, insurance verification, and delivery logs, ensuring accuracy and compliance
- Build and maintain strong relationships with hospital personnel case managers, nurses, physicians, and social workers
- Assist with quality improvement programs and compliance with national healthcare standards
- Provide in-services and training sessions as needed for healthcare professionals
- Ensure all company vehicles, equipment, and procedures meet safety standards
- Respond to urgent service needs and assist with resolving equipment-related issues
- Travel locally (using a company vehicle) to patient homes for follow-ups and reoccurring deliveries
- High school diploma or GED required; additional medical or clinical education preferred
- LPN, CNA, or EMT certification strongly encouraged to apply
- 3+ years of experience in healthcare, case management, or patient support (preferred)
- Familiarity with medical equipment, terminology, or billing/reimbursement processes (preferred)
- Valid driver s license with clean driving record (required)
- Strong interpersonal skills and ability to work collaboratively with medical teams and patients
- Excellent communication, organization, and problem-solving abilities
- Ability to manage sensitive patient information with discretion and professionalism
