Schedule: Tuesday Saturday 9AM-5:30PM with flexibility

Overview:
Make a lasting difference in patients lives by ensuring a smooth transition from hospital to home. As a Hospital Liaison, you ll work closely with hospital case managers, physicians, and care teams to coordinate home healthcare services for patients who rely on respiratory and medical equipment.


You ll play a hands-on role in arranging timely deliveries, educating patients and caregivers, and ensuring they have the resources to manage their health safely and confidently at home. Responsibilities:
  • Collaborate with hospital case management and continuing care teams to plan and coordinate discharge for home-based services
  • Educate patients and caregivers on proper use, safety, and maintenance of respiratory and medical equipment
  • Facilitate the delivery and setup of equipment at the patient s point of discharge
  • Manage all required documentation, including intake forms, insurance verification, and delivery logs, ensuring accuracy and compliance
  • Build and maintain strong relationships with hospital personnel case managers, nurses, physicians, and social workers
  • Assist with quality improvement programs and compliance with national healthcare standards
  • Provide in-services and training sessions as needed for healthcare professionals
  • Ensure all company vehicles, equipment, and procedures meet safety standards
  • Respond to urgent service needs and assist with resolving equipment-related issues
  • Travel locally (using a company vehicle) to patient homes for follow-ups and reoccurring deliveries
Qualifications:
  • High school diploma or GED required; additional medical or clinical education preferred
  • LPN, CNA, or EMT certification strongly encouraged to apply
  • 3+ years of experience in healthcare, case management, or patient support (preferred)
  • Familiarity with medical equipment, terminology, or billing/reimbursement processes (preferred)
  • Valid driver s license with clean driving record (required)
  • Strong interpersonal skills and ability to work collaboratively with medical teams and patients
  • Excellent communication, organization, and problem-solving abilities
  • Ability to manage sensitive patient information with discretion and professionalism
